This time presenting with yet another staggering weblog giving an idea about THE EXTREME GUM(GINGIVA) INFECTION AND OTHER SUPPORTING STRUCTURES INVOLVEMENT CALLED AS  DENTAL ABSCESS.

CLOSE VIEW OF THE DENTAL ABSCESS:-(CLINICAL VIEW).

Do let’s understand the DENTAL ABSCESS. 

DENTAL ABSCESS IS THE COLLECTION OF THE PUS UNDERNEATH THE TOOTH (ADJOINING STRUCTURES)OR THE GUMS(GINGIVA).

ONSET OF INFECTION-COMMENSURATES AS A untreated decay(caries),injury(sports injury/fall),leak from old dental work.(artificial  removable/permanent prosthesis-crowns/removable dentures /fillings/rct /ill-fitting prosthesis etc.),cracked teeth/broken/fractured teeth etc.

BASIC SHOW-UP OF THE NORMAL AND ABSCESSED TOOTH :

Dental abscess starts as in STAGES:

STAGES CAN BE DIVIDED AS BELOW:-

INITIAL- this stage doesn’t hurt in any way as the infection is restricted to only enamel and the dentin.(SUPERFICIAL STAGE not involving the PULP).

ADVANCED-this stage goes deep or penetrates into the pulp causing SENSITIVITY INITIALLY THEN PAIN IN ACTUAL .IF THE PAIN PERSIST WITH INFECTION EXHIBITING AS FURTHER SPREAD DOWNBELOW INVOLVING BONE,GUMS AND OTHER TEETH- THE PUS/FLUID SEEN/VISIBLE AS A GUM POP(ABSCESS)/SWELLING.

TYPES OF DENTAL ABSCESS:-

Abscess can appear around the the tooth in relation to the gums and the supporting structures.

 

DENTAL SURGEONS ROLE:-

Here the ROLE OF THE DENTAL SURGEON STARTS -TO BEGIN WITH-

RADIOLOGICAL INVESTIGATION -IOPA/FULL MOUTH X-RAY(OPG).

CLINICAL EXAMINATION-THOROUGHLY VISUALIZED THE STATE OF SURRROUNDING TEETH AND STRUCTURES.

Reasons for occurrence of abscess:-

A diet that is too sugary promotes the bacterial growth in the abscess.

Broken or chipped and cracked tooth/ teeth for infection. 

Sensitive teeth having tiny cracks allow bacteria to penetrate through enamel to the  dental pulp.

Symptoms of the gum disease-bleeding and soreness.

Wisdom teeth infected around.

Dental procedure causing infection.

Injury to the teeth.

 

SYMPTOMS OF DENTAL ABSCESS:-

Depends upon the spread of infection..

PROPAGATION OF ABSCESS IN STAGES:-

1) EARLY STAGE:-

Expressed as- sensitivity to hot and cold temperatures. 

             -headaches

             -pressure to chewing and biting

             -bad breath(HALITOSIS).

2) LATER STAGE:-

WHEN THE INFECTION GETS BLOWN OUT OF PROPORTION SND ITS SERIOUS REPERCUSSIONS. 

-throbbing pain radiating to head(ear) ,jaw and neck.

-swelling of the face and cheek.

-swollen lymph nodes under jaw.

-abscess can appear in the form of bump on the GUM..

-pain while chewing and biting.

-Fowl breath(HALITOSIS)and taste(DYSGUESIA)

-teeth will feel  especially sensitive to pressure and temperature.

-pus may ooze between infected tooth and gum line.

Discolored or loose teeth.

 

PATIENTS WHO ARE MORE SUSCEPTIBLE TO TOOTH INFECTIONS :

Smoking and alcohol intake. 

PATHOLOGICAL CONDITIONS :Dry mouth(XEROSTOMIA).

INFECTIONS FLARING UP -OWING TO UNDERLYING SYSTEMIS DISEASES LIKE DIABETES,HEART AILMENTS, RESPIRATORY, ARTHRITIS (RHEUMATOLOGICAL) ETC.AND MANY MORE

Poor dental hygiene.

Weakened immune system causing immunodeficiency diseases like CANDIDIASIS ETC.

Diet high in SUGARY DIET.

Medications- excess use.

Null ACTIVITY/EXERCISE. 

Time to PONDER!!

If the lifestyle  and underlying systemc illnesses not considered and  modulated can and will  result in mild -moderate to severe infections in the mouth affecting the routine livelihood. 

TAKE INTO ACCOUNT TO PUT A CURB ON ABOVE MENTIONED HABITS AND CONDITIONS TO BE EVALUATED TIME TO TIME. 

 

TREATMENT MODALITIES UNDERTAKEN BY THE DENTIST:-

INITIAL TREATMENT:- INCLUDES A COURSE OF ANTIBIOTICS FOLLWED BY INCISIONAL DRAINAGE AFTER THE FLARED-UP INFECTION GETS UNDER CONTROL.

GRADUALLY,  FURTHER INVESTIGATIONS ARE CARRIED OUT:-

Primarily the X-RAY IS TAKEN.

THAT’S EITHER -IOPA(LOCALIZED RADIOGRAPH).

                             -OPG(FULL MOUTH RADIOGRAPH.

DEPENDING UPON THE LOCATION AND SEVERITY OF THE INFECTION. 

BY STUDYING THE FINDINGS  on the RADIOGRAPH-

TREATMENT/FINAL COURSE  IS DECIDED:-

                    EITHER 

ROOT CANAL TREATMENT -in order to save the tooth if possible. 

                         OR

TOOTH EXTRACTION -last resort undertaken if the condition of the tooth is completely deteriorated.

SPACE INFECTION SPREADING TO HEAD AND NECK.

Note:-If ignored, very serious abscess can affect the floor of the mouth and neck.THIS NEEDS TO TREATED SURGICALLY.
