giving you all an idea about my New Blog based on POST AND CORE PROCEDURE DONE POST ROOT
CANAL TREATMENT.

about the POST AND CORE:
POST AND
CORE:- IS THE DENTAL RESTORATIVE PROCEDURE MEANT FOR ATTAINING THE LONGEVITY OF
THE TOOTH HAVING COMPROMISED (MINIMAL) TOOTH STRUCTURE IN THE ENDODONTICALLY
TREATED TEETH AFTER THOROUGH EVALUATION OF THE TOOTH CONCERNED.
STABILITY,
LONGEVITY AND STRENGTH ARE ALL FULFILLED WITH THIS PROCEDURE KEEPING ALL THE
LIMITING FACTORS IN MIND FOR A SUCCESSFUL OUTCOME.
CONDITIONS
THAT FAVOUR THE PRIME REQUIREMENT OF THIS PROCEDURE FOR ATTAINING THE COMPLETE
STABILITY AND LONGEVITY WITHOUT LOOSING THE TOOTH ARE:-
EXCESSIVE
TOOTH STRUCTURE LOSS DUE TO HEAVY DECAY(CARIOUS).
OLD WORN
-OUT RESTORATION (FILLINGS).
PREVENT RE-INFECTION
IN THE ROOT CANAL TREATED TOOTH BECASE RCT RESULTS IN REDUCTION OF THE ELASTICITY
AND INCREASED BRITTLENESS OF THE TOOTH.
PREVENT THE
REMAINING TOOTH STRUCTURE FROM THE FRACTURE OR ANY TRAUMATIC VULNERABILITY.
EXPLAINATION
FOR POST AND CORE:
POST/DOWEL:-It's a metal rod placed in the root of
the tooth TREATED with ROOT CANAL TREATMENT TO AID IN RESISTANCE TO THE
EXTERIOR FORCES. (MASTICATION/TRAUMATIC
IMPACT).
CORE(BUILD-UP
RESTORATION):
Material
used :-TOOTH COLORED RESTORATION (COMPOSITE),NON -TOOTH COLORED (AMALGAM) RESTORATION.
OTHERS
ARE:-
CAST /METAL
CORE,GLASS IONOMER CWMENT(GIC),TESIN MODIFIED GIC RESTORATIVE MATERIAL.
CORE:-is
placed in the mouth of the crown part of the tooth or projects over the
gums(gingiva) that will in turn support or anchor the ARTIFICIAL CROWN/CAP
AIDING IN RETENTION AND PROVIDING PROTECTION TO THE WHOLE TOOTH AS A A SOLE ENTITY).
SAVIOUR FOR THE DAMAGED TOOTH :-
Both POST
AND CORE ARE BUILDING-UP THE TOOTH STRUCTURE FOR THE FUTURE RETENTION LIKE
DIFFERENT CROWNS FOR AESTHETICS AND FUNCTIONAL ACHIEVEMENT(FINAL RESTORATION/FOUNDATION FOR THE ARTIFICIAL CROWN).
Types:
CUSTOM
-MADE (FABRICATED ACCORDING TO THE TOOTH STRUCTURE ON A WHOLE AVAILABLE IN THE
DENTAL LAB).
PRE-FABRICATED
which are ready- made and can be given immediately
(IMMEDIATE- POST AND CORE), POST RCT if the fit of the POST and CORE is favourable
and adaptable with the tooth structure present.
POST AND
CORE MATERIALS:-
Custom/prefabricated:-
CAST ALLOYS
POST / METAL POST :-Gold alloy,Titanium,Chromium-cobalt alloy,Nickle -chromium alloy,stainless
steel, brass etc.
NON CAST /NON
-METAL POST:-Ceramic and Zirconium,FIBRE REINFORCED RESIN eg: carbon/glass fibre,
quartz,glass and silicon etc.
POST AND CORE:- CAN ALSO BE ACTIVE OR PASSIVE.
PRE-
TREATMENT EVALUATION:-
Endodontic
approach:
To look for
the response of the tooth after RCT(ROOT CANAL TREATMENT).⁹
Any surrounding
structure, inflammation or swelling causing any Abscess/Pus formation that will
surely limit going ahead with the POST AND CORE.
Periodontal
approach.
Looking for
the gingival health and bone structure to be sound enough favoring the POST AND
CORE TREATMENT.
Biomechanical
evaluation:-
Includes amount
of tooth structure present .
Placement
of the tooth(Anterior(front) and (Posterior(back) – Masticatory (occlusal
forces),
RECURRENT Caries
Injury to
the tooth(fracture) etc.
Anatomic
evaluation:-
Root
evaluation :
Root curvature ,root anomalies(Deformaties) etc.
CROWN EVALUATION:-
Crown part structure
to be evaluated (in terms of tooth
material available that will favor the POST AND CORE PROCEDURE).
CONTRA- INDICATIONS:-
Severe
curvatures of the roots called DILACERATION.
Persistent
PERIAPICAL LESIONS.(infection at the
apex of the tooth. Like Abscess.
UNFAVOURABLE
Periodontal health(supporting structures of the tooth enabling the tooth
stability inside the bony socket).
Economic
status of the patient :-allowing or not allowing the patient to settle for the compromised material
in a longer run.
Inadequate
skills or missing expertise.
Teeth
bearing heavy occlusal forces/MASTICATORY forces.
IDEAL REQUISITES FOR POST AND CORE PROCEDURE :-
POST
REQUISITES:-
Should
provide support to the ROOT.
Should
provide retention within the root.
AESTHETICS
to be considered depending upon the POST placement.
Simple,safe
and reliable for both ROOT and the ARTIFICIAL CROWN.
Should
provide even distribution of the functional stresses.
Should
allow easy removal of the complete entity when required after prolonged periods
of CLINICAL USE.
It should
be made of BIOINERT MATERIAL resisting Corrosion and any other form of
DETIORATION IN THE MOUTH.
Should
resist displacement of the POST DUE TO EXTERNAL
STRESSES. (FUNCTIONAL/MASTICATORY/ CHEWING FORCES).
CORE REQUISITES:-
CORE part
should be stable in the wet environment.
Should imitate
the natural color of the tooth.
Adaptable
enough to hold th ARTIFICIAL CROWN.
BIOINERT-non
corrosive.
BIOCOMPATIBLE
with the tissues and the oral environment(ORAL flora(bacteria)..
Cost for
the material. Can at times become limiting factor for the patients to
select,thus compromising with their pocket expenses
PROCEDURE AND
STEPS INVOLVED FOR INCORPORATING POST AND CORE:-
Perform RCT (ROOT CANAL TREATMENT).
INSERT THE
POST INTO THE CANAL OF THE ROOT.
BOND THE
POST TO THE CANAL OF THE ROOT.
BUILD -UP
OF THE SUITABLE MATERIAL AROUND THE CORE
CONSIDERING AESTHETICS AND FUNCTION.
FIX THE ARTIFICIAL CROWN OVER THE CORE BUILD-UP PORTION.
MAIN
OBJECTIVE OF THIS TREATMENT IS TO ATTAIN FULL OCCLUSAL(MASTICATION) AND
COSMETIC FUNCTION.
POST AND
CORE ARE ESSENTIAL FOR REINFORC8NG THE TOOTH AND CREATING A FIRM BOND FOR A
DENTAL CTOWN THAT LASTS.
POST AND
CORE SYSTEM HELPS IN ACHIEVING INTERIOR AND EXTERIOR SUPPORT FOR THE WEAKENED
TOOTH.(IN TERMS OF MECHANICAL,COSMETIC AND FUCTIONAL REASONS).
Difference
between an IMPLANT AND A POST AND CORE PROCEDURE :-
IMPLANT :- Is a structure that replaces the root of the missing natural tooth.
It’s just a
screw inside the bone that with time fuses with the bone tissue.( behaves as a
root).
It helps in holding the crown (Cap),bridge(fixed - partial or complete teeth rehabilitation (PROSTHESIS ) of the mouth-ARTIFICIAL PROSTHESIS.
PRE-FABRICATED
OR CUSTOM-MADE POST:-
It is a
metal pin placed in the highly decayed tooth healthy enough to hold it.
The natural
tooth root is preserved.
Then a
crown/bridge can be affixed to the POST.
Note: POST
IS RECOMMENDED WHEN A ROOT CANAL HAS BEEN PERFORMED AND NOT ENOUGH NATURAL
TOOTH REMAINS TO HOLD THE ARTIFICIAL PROSTHESIS/RESTORATION.
THREE CRUCIAL POINTS THAT FAVOUR IN GOING FOR THE POST AND CORE PROCEDURE ARE:-
If the
dentist BOND the crown directly to the heavily decayed tooth,tooth can further
break and weaken.If the break is severe or severe infection picks up ,in that
case one caboose the tooth.
Then comes
the role of DENTAL IMPLANTS FOLLOWED SUPPORTED BY CROWNS AND BRIDGES(FIXED
PROSTHESIS) ON THE TOP.
That

This has been explained by the auther in very simple language for everyone to understand this modality of Dental treatment. Very nicely covered this difficult topic.
